Keystone’s unique approach delivers comprehensive care to patients who are in need of post-acute care but don’t have access to care or transportation. The burden placed on hospitals can be alleviated with our ongoing primary and specialty care for those with complex medical conditions. Continuous monitoring of patient conditions allows situations to be assessed and treated before it becomes urgent.
Our house call services include:
- Designated health team (consisting of primary care provider and care
manager) for each patient - Primary care, follow-up, post-acute care, and wellness visits
- Specialty care including psychiatric/mental health, Internal Medicine and
- Pulmonary Medicine
- Coordination of in-home services including: pharmacy delivery, In-home
laboratory draws, and diagnostic testing - Medication management
